Thiopurine methyl transferase (TPMT) is a cytoplasmic transmethylase enzyme found in mammalian species. TPMT catalyzes the S-methylation of aromatics and heterocyclic sulfhydryl compounds, including medications, such as mercaptopurine, thioguanine, and azathioprine (Elion, 1989).
The TPMT, RBC assay is used as a screen to detect individuals with low (abnormal) TPMT activity that may be at risk for excessive myelosuppression when exposed to standard doses of thiopurines such as azathioprine (Imuran) and 6-mercaptopurine (Purinethol). Thiopurine drug therapy is used for autoimmune diseases, inflammatory bowel disease, acute lymphoblastic leukemia, and to prevent rejection after solid organ transplant. The inactivation of thiopurine drugs is catalyzed in part by thiopurine methyltrasferase (TPMT) and nudix hydrolase 15 (NUDT15). Thiopurine drugs are used most often for autoimmune disorders, such as rheumatoid arthritis. It is also commonly used for organ transplant recipients and when a diagnosis of Crohn’s disease has been confirmed.
